There is a new, fragile possibility for the two-year-old boy hospitalized at the Monaldi Hospital in Naples. Yesterday evening a new heart became available, but before any decision the evaluation of the multidisciplinary team convened for today will be decisive. The mother, Patrizia Mercolino, was called urgently to the hospital: no statement to reporters, while hope returns to make room alongside clinical prudence.
The little boy is in serious but stable condition, supported by an extracorporeal circulation system that allows him to stay alive after the failed transplant on December 23rd. That heart, which arrived already compromised, never worked. Now the word passes to the specialists who will have to evaluate blood type compatibility, general conditions, infectious risk and real possibility of facing a new surgery.
The maxiconsultation and the clinical issue
A real Heart Team is expected at Monaldi with experts from some of the most authoritative pediatric facilities in Italy: the Bambino Gesù Pediatric Hospital, the University Hospital of Padua, the ASST Papa Giovanni XXIII of Bergamo and the Regina Margherita hospital of Turin. A collegial evaluation, at the patient’s bedside, to establish whether there are concrete margins for attempting a second transplant.
The clinical picture remains complex: cerebral hemorrhage, signs of multiple organ failure, an infection that is difficult to control. Elements that make every choice a very delicate balance between risk and possibility. Doctors will have to establish whether the child’s body can handle another operation and the subsequent immunosuppressive therapy, which is essential after the implant.
The investigation into the damaged heart
At the same time, the investigation by the Naples Prosecutor’s Office continues. The transport of the organ taken from Bolzano is under scrutiny: the heart would have been kept in an old-fashioned container, without temperature monitoring systems, despite more advanced technological boxes being available. Among the investigative hypotheses, the use of dry ice instead of traditional ice which would have caused excessive cooling, compromising the organ.
Six people are under investigation for negligent injury; other hearings are scheduled in the coming days. Inspectors from the Ministry of Health are expected between Naples and Bolzano to verify procedures and responsibilities along the entire operational chain with the government ensuring its commitment to clarify the matter.
