An event of absolute scientific exceptionality has transformed a rare anatomical condition into a medical miracle. In Padua, a young 24-year-old woman of Egyptian origin, Manar Ismaili, gave birth to twins who grew up simultaneously in two separate uterine cavities. An extraordinary clinical case, previously documented only 6 times in the world scientific literature, which saw the birth of little Noah and his little sister Laila.
The rare anatomy and a double parallel gestation
The new mother lives with a congenital malformation known as a bicornuate-bicornuate uterus, characterized by the presence of two distinct organs, smaller in size than normal and with limited elasticity. The exceptional aspect of the story lies in the implantation and parallel development of two embryos within each compartment: on one side the boy and on the other the girl. The woman and her husband Abdelrahman entrusted themselves to the care of the Venetian facility from the first weeks of pregnancy, facing a very complex gestational journey.
Clinical management and surgical intervention in Padua
The patient was taken into care at the 16th week of gestation at the University Hospital of Padua, following a recommendation from the doctors of San Donà di Piave. The clinical picture required constant and rigorous monitoring, with biweekly visits coordinated by Dr. Paola Veronese, director of the Obstetrics and Gynecology UOC, also due to the type 1 diabetes mellitus from which the patient is affected. The presence of the two less elastic organs exposed the gestation to high risks of complications.
Cesarean section birth and conditions of newborns
At the 30th week, the premature rupture of the baby’s membranes required emergency hospitalization. The medical team administered the necessary therapies to accelerate the lung maturation of the fetuses, performing the cesarean section 2 days later, on September 23, 2026. The surgery involved a single incision to sequentially access both cavities: Noah was born with a weight of 1,930 grams, followed by Laila with 1,590 grams. The two children are in excellent health and are completing the final stages of care in the Neonatology department in view of their upcoming discharge.
